I authorize Castilleja School personnel to release my transcript and GPA to colleges, universities, scholarship, athletic, internship and summer programs. Pursuant to the State of California Civil Code, Sections 1633.1-1633.17, by typing in my name in the name block below, I am electronically signing and consenting to the provisions of the form above. Further, in electronically signing, I certify that I am an alumna, a student or a parent/legal guardian of the student. The student or the parent/legal guardian of the student agrees that the above authorizations and consents shall continue in effect from year to year as long as the Student is a student at Castilleja School, unless I subsequently notify the Head of School in writing that these on-going authorizations and consents have been terminated.
